Preinduction Cervical Ripening


Abstract
The main function of the cervix is to keep the uterine cavity closed for a period sufficiently long to prevent preterm birth. The endpoint of cervical alterations, normally reached between 38 and 42 menstrual weeks, is a ripe, favorable, or ready cervix.
If conventional labor induction by low amniotomy and intravenous oxytocin is attempted while the cervix is still unfavorable, first stage labor will often be protracted and abdominal delivery will be indicated in an unacceptably high number of cases.
Therefore, the cervical state is the main predictor of both the time of onset of labor and the outcome of conventional induction of labor.
Until recently, the effectiveness of surgical induction was restricted by the presence of an unripe cervix. At present, since sustained release of natural prostaglandins(PGs) administered via various routes was shown to be highly effective for priming the cervix, this problem is no longer a major obstacle for conventional labor induction.
The purpose of this study is to review the evolution of ripening procedures and to discuss effective techniques. Consequently, we shall concentrate on methodological aspects and the clinical results of pre induction cervical ripening.
